OLYMPIA, Wash.-The Washington State Department of Health has suspended the license of a Nursing Assistant in Franklin County charged with patient neglect at an adult family home.

The DOH’s Nursing Assistant Program issued a notice of charges against Yesenia Adi Chavez on June 8 and she was allowed time to respond legally.

In September Adi Chavez’s Nursing Assistant License was suspended pending further legal action by the state Nursing Assistant Program.

Adi Chavez was certified as a Nursing Assistant in 2011 according to the DOH, and the charges stem from cases of neglect in 2021 and 2022.

In 2021 a cancer patient at Adi Chavez’s adult family home was hospitalized with sepsis from poor wound care and later died after returning to the care facility.

According to the DOH, another patient of Adi Chavez’s was hospitalized for 23 days in 2022 to be treated for extensive wounds as a result of neglect.
